The official synopsis for the season 5 finale titled "return 0" reads:

"The team must embark on one last suicide mission to prevent Samaritan from destroying The Machine and cementing its hold over mankind."

In an interview with TV Line, co-showrunner Jonah Nolan explained how they came up with the finale episode title. "The working title of the episode — before we settled on 'return 0,' which is a vestige of my old days as a coder — had been, 'Everyone Dies Alone,' which reflected our mood at that moment," he said. Co-showrunner Greg Plageman went on to add, "We thought that we was too upbeat!"

As seen in the promo photos, most of the scenes were set in the team's underground base. It turns out that The Machine was actually the one narrating in the pilot, and that the lair will be destroyed once everything has been said and done.

In spite of the show wrapping up its storyline, there might be a possibility of a spinoff series happening. As TV Guide pointed out, this week's episode "Synecdoche" seems like a "possible backdoor pilot to extend the life of the franchise." Episode 17 clearly left a lot of questions that can lay the groundwork of a new show, like how many teams are under the Machine's control and when did they begin existing? Is a "Person of Interest: SF" or a "Person of Interest: Windy City" possible? Was it the original plan to establish Logan Pierce as a wealthy person only to make him Finch's alternate? The list goes on but the execs have yet to announce if a spinoff is on the table.
